The traditional Chinese medicine Rehmanniae Radix, a fresh or dried root tuber from the Scrophulariaceae family plant Rehmannia glutinosa Libosch, is widely used as an herbal remedy. This ancient TCM compound has been credited with various biological effects such as analgesic, sedative, anti-inflammatory, antioxidative, antitumor, immunomodulatory, cardiovascular and cerebrovascular regulation functions, and nerve regeneration promotion abilities. Recent studies have explored the chemical constituents and pharmacological activities of Rehmanniae Radix RR, shedding light on its diverse therapeutic potential.
Key active ingredients identified in RR include iridoid glycosides like catalpol and aucuboside, phenylpropanoid glycosides such as acteoside, other types of saccharides, and unsaturated fatty acids. Moreover, the Chinese patent medicine CPM related to Rehmanniae Radix has also garnered significant attention among TCM practitioners; notable examples include Six Drugs Bolus, which includes Rehmannia, Lily Bulb, and Rehmannia Decoction, as well as Siwu Decoction. reviews recent literature on RR, summarizes its chemical composition, biological functions, and clinical uses, analyzes current research progress and limitations to provide insights for future exploration and development of this medicinal herb.
Rehmanniae Radix's pharmacological effects are rooted in its complex chemical profile that includes iridoids, which exert diverse beneficial actions by modulating cell signaling pathways. Iridoid glycosides have been linked with pn relief, anti-inflammatory properties, and antioxidative activities. The bioactivity of these natural products is mediated through multiple mechanisms involving the regulation of pro-inflammatory cytokines, modulation of immune responses, and protection agnst oxidative stress.
In terms of clinical applications, Rehmanniae Radix exhibits potential benefits in managing conditions like cardiovascular diseases, neurological disorders, inflammatory bowel disease, and depression, as evidenced by various studies. Its therapeutic potential is further supported by its traditional use in Chinese medicine for the treatment of these lments.
The review discusses ongoing challenges including the optimization of extraction methods to maximize bioavlability, elucidation of pharmacokinetic profiles, and understanding of interactions with other medicinal compounds when used in combination therapies. Additionally, efforts are being made to standardize quality control measures for RR-based products to ensure consistent efficacy and safety.

Further interdisciplinary research integrating traditional knowledge with modern analytical techniques is essential to unlock its full potential in contemporary medicine.
